The Internal Revenue Service (IRS) has made a minor change to Form 8809, Application for Extension of Time To File Information Returns — they will no longer be accepting faxes of this form.
Additionally, to submit Form 8809 on paper, it must be mailed to the IRS location in Ogden, Utah at:

Use this form to request an initial or additional extension of time to file only the forms shown on line 6 of Form 8809 for the current tax year. Extension requests are not guaranteed for approval by the IRS.
The IRS encourages tax preparers to submit forms electronically to avoid delays.
